REQUEST FOR PROPOSAL
CUSTODIAL MONITORING SERVICES
(Independent Contractor)
Various Municipal Buildings
SCOPE OF WORK
The scope of work for the project involves custodial monitoring services at various municipal buildings and locations. The contract will commence in March 1999. Subsequent one (1) year renewals may be available for two (2) additional years at the sole option of the Town of Enfield. The total contract period including all options shall not exceed 36 months.
SUBMISSION REQUIREMENTS
Submit written statements and/or resumes in response to items listed below:
Custodial or related supervision experience - a general list of locations for which the individual has provided related supervision, indicating project title, owner’s name and telephone number, location, size, description and completion date. Include a description of individual’s involvement in these projects.
Resume – a resume of all pertinent information including previous work experience and training as it relates to custodial services or supervision.
References – a complete list of references which note an owner representative’s name and phone number.
FEE
The projected fee should be based on a monthly payment schedule. A fee of $1,000.00 per month will be paid to the selected monitor. It is expected that the hours required to provide these services will be from 7 P.M. to 11 P.M. Monday thru Friday; although additional time may be required and those charges are expected to be included within the original base fee. All costs and expenses associated with the project must be included in the monthly fee. The monitor selected for this project is responsible for own Federal/State taxes.
SUBMISSION DEADLINE
Three (3) copies of all Requests for Proposal must be submitted in a sealed envelope clearly marked "Request for Proposal for Custodial Monitoring Services". If forwarded by mail or courier, the sealed envelope must be addressed to "Director of Finance, Town of Enfield, 820 Enfield Street, Enfield, CT 06082."
INSURANCE
The bidder awarded this bid must provide a current Certificate of Insurance to the Director of Finance PRIOR to commencement of work, with the following requirements:
Liability limits for bodily injury and personal injury, $1,000,000 per occurrence.
Liability limits for property damage including that caused by motor vehicles, $1,000,000 per occurrence.
Contractual liability, $1,000,000 per occurrence.
Owner’s protective liability and property damage, $1,000,000 per occurrence.
Worker’s Compensation, as required by Connecticut State statutes.
The "Town of Enfield" is to appear as an additional insured on all Certificates of Insurance.
All insurance is to be provided by a company authorized to issue such insurance in the State of Connecticut.
All insurance may not be canceled or modified without sixty (60) days written notice by registered U.S. Mail to "Director of Finance, Town of Enfield, 820 Enfield Street, Enfield, Connecticut 06082."
SELECTION PROCESS
All submissions in response to this RFP will be reviewed by a selection committee. The selection committee may at its discretion hold interviews prior to making a final selection. A decision by the selection committee will be based on submissions’ qualifications, ability to facilitate and reference checks.

Sealed proposals for Custodial Monitoring Services for various municipal buildings will be accepted at the office of the Director of Finance until 11:00 A.M. , Monday, March 1, 1999. Thereafter, Proposals will be opened in public.
RFP Documents may be obtained from the Assistant Director of Public Works at the Jablonski Public Works Complex, 40 Moody Road, Enfield, Ct. 06082; (860) 763-7561.
The Town of Enfield reserves the right to reject any or all proposals to waive any informalities and to award the proposal deemed to be in the best interest of the Town.
